Works on Council Land Application Form

 

 

 

When working on Council or public property, you are required to inform Council of project intentions. Use this form to apply to Council for approval to carry out works on Council land, including trees and planting, pathways, gardens, small building projects etc.
Works on Council Land Acquittal Form

 

Use this form to notify Council once a committee has completed an approved project on Council land.

Financial Management
Monthly or Quarterly GST Returns Template

The information in this form is used to prepare Council's BAS Statement. If GST on income is greater than GST on expenditure, then the committee will be required to pay the net GST to Council.  If GST on expenditure is greater than GST on income Council will refund the net GST to the committee.

Please complete this form, at least, on a quarterly basis, even if it is a Nil return.
Annual Financial Statement Template
The information in this form is used in Council's Annual Financial Statements which are prepared on a financial year basis. Please complete this form within 2 weeks after the end of the financial year. This should be accompanied by a copy of bank statements for the period. This template may be used as a guide only.

Use this form to report incidents that concern your committee or have occurred at a facility your committee operates. You can use this form to report the following incidents:

Personal injury, Council property damage, public property damage, damage to utility, environment, theft, motor vehicle accident, near hit/miss, exposure to substance, abuse/harassment, unsafe condition and other.

Section 355 Committees Guideline

This Guideline aims to assist Section 355 Committees in understanding their roles and responsibilities and to ensure the effective and valuable partnership between Council and these committees remains well into the future.

Section 355 Committees Conflict of Interest Disclosure Form

Use this form to disclose an actual or perceived conflict of interests. Once completed, this form needs to be provided to your Committee Coordinator for further assessment.

You are offered a gift or benefit of token value($0.01-$10)

You can accept the gift/benefit and you do not need to complete the declaration form.

You are offered a gift or benefit of token value($10.01-$49.99) but you cannot refuse it or return it.

You can accept it and complete the declaration form, or surrender it and complete the declaration form.

You are offered a gift or benefit of token value($10.01-$49.99)and you have accepted it.

You need to complete the declaration form and send it to your Committee Coordinator. Be mindful that the gift or benefit cannot exceed more than $50 value in a 12-month period from the same person or organization.

You are offered a gift or benefit of token value ($10.01-$49.99) but you have refused it.

You need to complete the declaration form.

You are offered/given a gift or benefit of token value but it exceeds more than $50 value in a 12-month period from the same person or organization

You must refuse/return it if you can and complete the declaration form. Otherwise, you will need to surrender the gift/benefit to your Committee Coordinator who will need to pass it onto Council’s Public Officer and complete the declaration form.

You are offered a gift or benefit or more than token value (>$50).

You must refuse it and complete the declaration form.

FAQ's

What should I do with historic paperwork and financial information?

Any paperwork should be retained by the Section 355 Committee or Council as prescribed by the State Records Act 1998 (NSW). The State Records Act provides a number of means to lawfully dispose of State records. Generally, this is through the retention and disposal authorities issued by NSW State Archives. To find out more, contact your Committee Coordinator.

Does Council provide automated external defibrillators (AED) at facilities?

Whilst Council does not supply AEDs at community facilities, grants are available for Section 355 Committees and regular hirers of Council’s facilities to purchase AEDs. Ongoing servicing and maintenance is the responsibility of the group who purchased the AED.

Are WHS inspections undertaken at Council facilities managed by Section 355 Committees?

Council undertakes a number of inspections when undertaking planned maintenance and routine inspections. Fire safety and electrical equipment is tested as per relevant Australian Standards and building codes etc.
